Billing
Out-of-Network Provider
Crestview is not a preferred provider with insurance carriers. Full payment for the visit is required at the time of service. We are able to bill your visits as an out-of-network provider to your insurance company and any payments will be handled as a refund or credit to a future visit. This is a no-cost service that we offer to all patients, upon request.
Private Pay
Patients are required to pay at the time of service. We are happy to assist patients however we can with their billing needs.
Motor Vehicle Insurance and Workers Compensation
Crestview will bill on your behalf to your motor vehicle insurance carrier. If you have an Oregon policy, it is required to have personal injury protection, and that is what is used to fund your recovery from an accident. We handle the claims and billing so you don’t have to, and there is usually little, to no, out-of-pocket cost.
